Transition of Students with Disabilities

 


 

Region 4 provides resources designed to meet federal and state guidelines for transition services for students with disabilities.

Region 4 also works with Texas Transition Student Centered Transition Network to promote communication and collaboration between stakeholders and to ensure that all students have access to comprehensive transition planning.
 

Region 4 is available to provide technical assistance and training to districts, campuses, and charter schools in all areas of transition planning for students with disabilities.

 

Transition is designed to be within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child's movement from school to post-school activities, including:
  • Post-secondary education
  • Vocational education
  • Integrated employment, including supported employment
  • Continuing adult education
  • Adult services
  • Independent living, or community participation
  • Development of employment and other post-school adult living objectives
  • If appropriate, acquisition of daily living skills and provision of a Functional Vocational Evaluation
     
Postseondary Transition planning for students with an Individual Education Program (IEP) is both federally and state mandated.
