Greetings from the Region 4 Transition Listserv! My goal is to bring you the latest in transition updates, share resources related to transition planning and assessment, and provide you with current transition events in our area.
 
Do you have an upcoming transition event in your district that you would like to share with other parents, students, and/or educators? Share it with me and I'll pass it along.
 
Pat yourself on the back for another school year and hopefully another Indicator 13 and Indicator 14 season almost done! As you are working through all of the challenges with supporting your students who are graduating, please reach out and let me know if you need assistance. Transition Planning Guide for Students in Foster Care
A new Transition Planning Guide for Students in Foster Care Receiving Special Education Services is now available. The child welfare and education systems are charged with helping older youth transition to adulthood. This is especially true for students with disabilities.

While each system has extensive transition planning policies and procedures, the processes often work in parallel, but not in coordination with each other. The new guide clarifies the similarities and differences in the transition planning processes in the child welfare and special education systems. The guide also includes best practices and recommendations for improved coordination between systems to better support students with disabilities in foster care during the transition to adulthood.
